The "Parental Consent Form" is required by Awana HQ.  Click on the this form shown above and print one for each of your clubber participants.  It is the responsibility of each church to bring the form signed by parents to the AwanaGames.  Any clubber who does not have a completed signed form cannot participate in the Games. 

This form is in addition to what you use for your church.
